Therapy goals should be individualized and functional. They should be created with consideration to each patient’s needs, motivation, interests, strengths and weaknesses.
However, sometimes a goal template or goal bank provides a useful framework for individualizing objectives. I think of goal templates as the ‘bare-bones’, basement floor or structural foundation that I use to build functional and individualized goals from. I don’t always use a template…but sometimes I’m scratching my head and need some guidance on a direction to take.
Here are some lists I’ve come across in the past that helped me with goal setting. I DID NOT create these goal lists…they were passed to me from a therapist who got it from another therapist…you get the idea. Additionally, these collections of goals from various electronic goal banks during my years of PRN. For this reason, you will see no branding, copyright or author cited on these 2 documents.
